Complete and email a separate form for each rider nominating. When you have entered your data, read the Riders Declaration and tick the rider declaration confirmation box. Then click the 'Submit' button.
Please note that this form will not be sent unless fully completed and the declaration confirmation field is ticked.
A confirmation email will be sent by the recipient.
